- Diagnosis and reason for referral
- Relevant funding details (NDIS number, DVA file number, claim number etc.)
- Client's availability and preferred appointment frequency
- Contact person for booking (if different from the client)
- Relevant medical history, reports or assessments

Assessment begins
The clinician completes an initial assessment and develops a care plan tailored to the client's goals.

We keep you in the loop
Progress reports go to the referring practitioner. We can coordinate care with your team as needed.
