For a smooth referral.
- Patient's full name and date of birth
- Diagnosis and reason for referral
- Relevant funding details (NDIS number, DVA file number, claim number etc.)
- Patient's availability and preferred appointment frequency
- Contact person for booking (if different from patient)
- Relevant medical history, reports or assessments

We receive your referral
Our admin team reviews the referral and confirms eligibility and funding within 1 to 2 business days.

We contact the patient
We reach out directly to the patient (or their carer) to schedule an initial appointment.

Assessment & treatment begins
Our clinician conducts an initial assessment and develops a tailored care plan.

Progress updates to you
We keep referring practitioners informed with progress reports and can coordinate care as needed.
